An Affidavit of Support I-864 form is required by U.S. law for immigrants to show they will have adequate means of support after immigration to the United States.
Each of the Affidavit of Support forms I-864, I-864W, I-864EZ, and I-864A have step-by-step instructions for completion by the sponsor or joint sponsor.
The National Visa Center will review the Affidavit of Support I-864, Affidavit of Support I-864EZ, Affidavit of Support I-864W or Affidavit of Support I-864A for completeness and correctness.
When a corrected Affidavit of Support I-864 form is accepted by the National Visa Center, it will be sent with the immigrant visa petition to the embassy or consulate where the applicant will apply for a visa.
The following immigration candidates need an Affidavit of Support I-864 form:
• Applicants for family based immigrant visas, including most orphans.
• Applicants for employment based immigrant visas in which a relative filed the immigrant visa petition or has at least five percent ownership in a business that filed the petition.
An Affidavit of Support I-864 form does not need to be notarized, the sponsor only needs to sign the document. However, by signing an Affidavit of Support I-864 the sponsor certifies under penalty of perjury that the information and documentation provided are true and correct.
